Thanks for posting this. Keep in mind that this form is old. Well, at least in Iowa it's not the one they use anymore. I tried to use it a couple of years ago and was told this. Like I said though, it's Iowa... and I'm not sure of other states.

This certainly is a good form to start with though.

I thought it could use some changes so I may take it to my PCP next week and ask her what she thinks about it. IMO it's better to have something to present to your doctor than to ask for a letter. I may get my ole cranky rheumy to even fill out one of these. lol
