People know that psychiatric advance directives exist, but there are usually so many questions about them. Here is an excellent site from Bazelon Center for Mental Health Law that is very informative.

If you want to view Bazelon's form directive, it is at the end of the entire article. Or you can view the form directives (or sometimes mental health powers of attorney depending on your state) for your own individual state by clicking on The National Resource Center on Psychiatric Advance Directivesin the very first sentence of the article--then you go to the left of the page (scroll down some) and click on "state by state info".
