Have any of your doctors ruled out Secondary dysmenorrhea?

If not, don't take "here take this" for the final answer.

Dysmenorrhea (severe, frequent cramping during menstruation) can be primary or secondary.

Primary dysmenorrhea usually starts a couple years after periods begin I believe. Usually the pain starts when the bleeding starts and lasts for about two days.

Secondary dysmenorrhea is caused by something else (like endometriosis, or other pelvic issues).

When I began having horrible cramping so bad I couldn't stand up straight in 2005 I went to my doctor. I was given Tramadol because I need a pain med that wouldn't make me sleepy.

They thought I had a kidney stone so they ran me through some times, abdominal ultrasound, MRI (I think it was but I could be wrong). They didn't find a kidney stone but instead found a uterine fibroid.

Shortly after they found the fibroid I went to a specialist OB/GYN. She wanted to wait and see for two months to see if it was getting bigger. If it was we would talk about removing it. Turns out we conceived before my follow-up appointment and so far I've not had AF (Aunt Flo AKA menstruation) return.

Good luck to you!
