Last week when I was getting my Botox for pain (not wrinkles) My pain doctor gave me the notes from the new pain clinic (rehabilitation place) I'll drop some notes the doctor included, geez it's four pages long.

He had the opportunity to see me with my husband today for a consult regard complex pain symptoms. I brought with me no imaging studies - well I wasn't told to bring any & I didn't have any recent ones due to insurance denial!

I'm a pleasant 45yr old right handed former financial brokerage blah blah blah

She was seen by several Neurologists - I only saw 2 - is that several?

allergies: Fentynal patch caused nausea, Hydrocodone poorly tolerated - I told him I vomited every 10 or 15 minutes for like a day and a 1/2 - if you consider that nausea?

Functionally: she is independent in self-care with some difficulty due to limited use of left upper limb.

She has had frequent falls attributed to dizziness & balance disturbance. She does have a cane for support - duh, does this mean I'm independent?

sleep disturbed because of pain. bladder & bowel blah blah blah

I'm alert & pleasant in a moderate dysphoric and anxious mood. Gee, ya think

Some of my tendon reflexes are +1 and many are 4+/5, that's about two paragraphs & I don't know what they mean exactly.

I do think there may be some elements of Fibormyalgia, this may of course be perpetuated by her inactivity & mood disorder blah blah blah

He is concerned about my POLYPHARMACY. I do see several specialists. He goes on to say about me attending a 2week in-patient chronic pain facility - He told me my insurance would never pay for that.

I can't be to much harder on the guy, he is actually concerned & that some of my meds overlap each other.

I guess the Temazepam & xanax - I see my psych this week & I'm going to see if I can ween off the Temazepam. The Xanax, I don't take that often. I took one on the weekend.

Let's not forget - Socially: I'm living with my husband in a ranch style home with a basement. She has two children their ages & goes on to say about me smoking.

I will really try & be more ready for my next visit, I'll bring all my past Mri's & I have a crapload of medical files - but why did the guy I'm going to now not send him a copy of my file

He does also state that I have chronic left sided pain syndrome with neuropathic features & he suspects additonal cervical pathology, pending mri - but I can only get a cat scan

this is the doctor that RX'd me the Low Dose Naltrexone - which is helping
