MDJunction to me

jpcrps"When I found MD Junction, I was in the beginning stages of RSD/CRPS. I was scared, lacked knowledge about the condition, and felt very alone.

MD Junction changed all of that for me. I found friendship and terrific information from people who had first-hand knowledge of this syndrome. It was and still is a big part of my life.

MDJ was my first step on the journey of grief; from denial to acceptance. I am now inspired to help others by sharing this amazing site and sharing my own experiences. I am very impressed that one forum site can provide hope and inspiration to people suffering from so many different conditions. I am proud to be a part of this community.

My husband, 60 y/o, was rushed to the hospital by ambulance due to weakness, jaundice, etc. Paramedics noticed his extended stomach, and asked if it was normal. He is quite slim.

He was jaundied, lethargic, and mentally out of it so to speak. He was dehyrated and during the hospital stay he recieved about thirty plus IV's w/potassium. His amomnia levels were high (84, latter rose to 94). While in the hospital they did a CT, 2 or 3 ultrasounds, numerous blood tests, and even a liver biopsy. The ascites became quite prominent with the IV's.

He was diagnosed with liver failure, Hepatic Encephalopathy after liver biopsy, also steatohepatitis with submassive necrosis, irregular islands of residual heptocytes showing marked fatty change are surrounded by young fibrous disease mixed acute and chronic inflmation, and condensed reticulin resulting from various unknown toxins. (Side note doc is checking for liver fluke due to our frquent travels to SE Asia).

Attending doctor (GI & Pathologist didn't agree with the pathology report. He performed a endoscpy where varices were detected. He told me my husband had advanced cirrohsis and had probably 2 years to a max of 7 years left IF he never drank and improved.

Doc then sent liver biopsy to a leading Liver Pathologist at UCSF (University California at San Francisco) which is a leading research hospital for further clarification. He told me a transplant was probable. The report came back that "there was a slight beginning of cirrhsis, fatty liver). OK this was an about face, or should I word it "save face" time foe the doctor.

Diet had been set to minimum animal protein, low sodium, etc. My husband is barely eating anything. Last visit doctor said it was OK for him to eat any meats.

It is very difficult to get him to eat or drink much water as he says he is full all the time.

Strangley the ultrasound and Metabolic blood panel results have not come back after 10 days. This is a large high rated hospital (top 100 Newsweek). Doc says "they weren't on his computer". OK, couldn't he call the hospital for the results? This is the third office visit with him. He also hasn't checked electrolytes UNTIL I demanded he do this. Blood work done the very next day.

He is going to double the diueretic after checking electrolytes as the ascites is not going down, although edema is less in ankles and feet. He has developed large red patchy areas on ankles and feet. Our doc says from lack of circulation. He is now using a cortisone cream which is helping.

Husband had an appointment w/our primary care doctor and wanted to know his amonia levels, and I said Dr. XYZ hasn't done any blood work to check that or electrolytes. AHEM! Just don't get it. This GI doc brags of his education, but does that really make him a good doctor??? Me thinks NOT! Now what? No liver specialists in our insurance covered group, and none closer than SF.

DH complains alot, snaps at me about "you don't expect me to eat all that do you?". Tat would be yelled with a 3 or 4 ounce serving of soup. He ate a few chunks of pineapple for breakfast, small amount of soup w/2 shrimp for lunch. Total calories by 4 PM today is less than 100, water less than 8 ounces.

What do I do????? He won't eat, says he can't. Weight is 178# Pre-hospitalization was 163#, went up to 183# with IV's. Normal was 175# for the previous 30 years or so.

Hope I don't sound like I am whinning........maybe I am...............

Dobra,

Don't worry about whining . You have every reason to be worried.

Although Liver Fluke is a possibility most of his symptoms do match but usually patients run a high fever (104)

He certainly seems to have symptoms of Cirrhosis which any liver disease can mimic.

If he doesn't drink they should be checking for other causes like Alpha 1, PBC, autoimmune. steatohepatitis Means inflammation caused by Fat in the Liver which is becoming a common cause of Cirrhosis. Often no cause will be found then it is called cryptogenic Cirrhosis.

You need to follow his . Bilirubin, Albumin, Creatinine, INR, Platelets,& Sodium.many Cirrhosis patients have problems eating. a few bites at a time are all that I can manage most of the time . Usually cold foods are easier for me. I freeze ensure ( For a frozen yogurt texture) and take a bite or 2 through out the day.Animal proteins usually increase the ammonia level. If he does eat meat they should be Chicken and Fish.

You can petition the insurance group to cover a hepatologist for him ( it usually goes to a review board and you may need a Ombusman or advocate to help you.many of the patients I know travel to a hepatologist for diagnosis ,setting up a treatment plan and transplant Seeing them yearly and seeing a local GI for everyday monitoring. If he goes for transplant he will have a hepatologist within the transplant team.

I would also suggest you peep in at the Cirrhosis Group.

You can request the results of his labs from the hospital. By law he is entitled to them. If they give you any trouble either contact a patient advocate or ask his primary to request them for you.

As to liver fluke, we both became ill within hours of a seafood (including raw) at Raffles Hotel in Phom Pinh. I was sick for over two weeks and still having problems for months after returning home. He was sick but nothing like me. Friend who is research doc for WHO says this often lays dormant for years. However, he did drink wine regularily for years. Obviously he quit after becoming so ill a few weeks ago.

He has never been sick before other than a common cold or flu bug, but not often. He was working 16+ days and commuting to the other coast weekly. In reality he worked 24/7 and has done that for years. Yes he was tired, so now is even more exhausted. He has been recovering from a fall due to hose across the walk way in the dark Sad (post concussion syndrome) so the symtoms morphed from that to this!

He is in a lot of pain, gasping and groaning alot grabbing his lower stomach. Maybe the lactolose? I don't know but he is in so much pain.

I am wondering about Stem Cell treatments for fatty liver, as this treatment seems promising. The more I read of liver issues the more confused I get especially without the esults of the dopplar ultrasound, metabolic blood panel, etc. How could it take so long for results? It has been 2 weeks now.

The GI doc wants to see him weekly. I'd feel better about the doc if he had other patients in the waiting room!

Raw sea food is so very dangerous for every one especially if his liver was already compromised with Fatty Liver.

The only thing I can say about stem research is to make certain that you talk to someone at a teaching hospital or someone that is involved in a certified study there are alot of scams out there right now with stem research.

I would be calling the Drs for the results. They should be able to tell you something by now. Please let me know what you find out.

Have no idea if he had any issues then, first signs of illness was maybe in Sept. when he wasn't eating as much.

The real culprit may be fresh water fish used in much of the cooking of Luang Prabang, Laos, NE Thailand, and Cambodia. It is fermented fresh water fish made into a sort of fish sauce. There is a tiny snail sometimes under the fish scales. It also is in the local and popular water cress in Luang Prabang-I knew of it and we didn't eat the watercress. Schistosomiasis can lay dormant for years in bile ducts and in the liver!UCSF is checking the liver biopsy for this now. This I am concerned about as we have often traveled to remote regions in Burma, Cambodia, Vietnam, Laos, Malaysia, and Thailand since 1987.

PS called the doctor's office for the 3rd time. I am told, we're "kinda busy today" "I'll try and give the msg to the doctor today". OK, what is notclear here? DH is out of meds, doc was to call in refill and or up the meds when he got the results of the blood work IE electrolites. Oh this same doc never requested amnonia levels after day 3 in hospital. DH was out of hospital after 8 days. It is now 30 days since he was released from hospital and has seen this GI doc 3 times, and still doesn't have lab results.

OK I am counting to calm myself down, after all this important doctor is so busy and "the results aren't on his computer".

I'd be calling the patient advocate Today and everyday until I got some answers.

I don't know how you're controlling yourself.

I'd stomp my feet for you if it would help
