crazy, crazy question I started thinking about after a recent discussion.

Lucky is the bp person that can take only one drug and live well and be happy. If Abilify, or Seroquel, or lithium can function as a stand-alone drug for this specific patient, wouldn't any of the other rainbow of bp drugs also successfully be considered as "stand-alone" if prescribed to a qualifying patient?

In other words, I wonder what the patient/bipolar diagnosis(es) criteria is that the FDA scores a given drug against. I guess there'd be other questions, too.

In more words, how political does this kind of thing get? Is it a question of how relative a drug is?

I say in advance, mia culpa — I meant no dissing to anyone. I be curious.

I know people just on Lithium for remission periods who only get extra stuff added on during episodes. I've seen studies of Abilify being used as monotherapy, but I've not seen them for Seroquel. The issue (to my untrained eye) being that while Lithium has both an anti-manic and anti-depressant effect, Seroquel only has an anti-manic effect as far as I remember.

I could be very wrong of course. Smile

Bipolar

Meds:

1200mg Lithium
10mg Abilify.
50mg Seroquel (for sleep)
0.5-1mg Rivotril (Klonopin) (PRN)
7.5mg Zopiclone (PRN)

abilify, seroquel, and lithium are named examples, but they are only representative (in my post) of all the other applicable drugs in bipolar treatment.

I'm not sure exactly what you're asking. All I know is that I'm only on the lithium (and a very rare Xanax). It's been that way since November 2009.

What works for some people won't work for others. I started out on just Lithium and a day later a low dose of Seroquel was added. Over the months and years meds were changed but I always had a moodstabilizer and at least one antipsychotic. Now I take 3 moodstabilizers and 3 antipsychotics all at once. When the mix fails I think my Seroquel XR will go up (if hypo/manic) and Lamictal up (if depressed). My sister is bipolar and only takes Zoloft and Abilify. She has bp2 and I have bp1 (a stubborn one). Not sure if that makes any difference.
Bipolar 1, Simple Partial Seizures, and migraines

Psychiatric meds:

Lithium ER (900 mg)*Tegretol XR (1400 mg)*Lamictal (100 mg)*Geodon (160 mg)*Navane (5 mg)*Seroquel XR (100 mg)*Klonopin (.5 mg)

Ativan 1 mg "as needed" (I rarely take it.)*Seroquel regular 50 mg "as needed" (Only if I'm getting elevated.)

Other meds:

Propranolol (40 mg)*Levothyroxine (150 mcg)

My mix is side effect friendly for me.

A stand-alone drug is a drug that can be used alone for the treatment of something.

For example, when Abilify first came out, it was approved by the FDA for the treatment of depression (and other things) WITH the addition of an addition of an antidepressant.

However, subsequent research has shown that Abilify ALONE is highly EFFECTIVE for the treatment of BIPOLAR disorder (and, apparently some other disorders - I'm not sure). Anyway, because of the research on bipolar, Abilify has been approved by the FDA as a "stand alone" treatment for bipolar. In other words, it can be used ALONE and is considered effective without the addition of an antidepressant or other medications for the treatment of bipolar - it is now a "stand alone" treatment.

Of course, we all know the effectiveness of bipolar treatments varies from individual to individual.

Bipolar II, Generalized Anxiety Disorder, Panic Attacks, Agoraphobia & PTSD; Alcoholic in recovery since 3/21/07.

Aplenzin 522mg; Pristiq 200mg; Lamictal 400mg; Geodon 160mg; Ativan 1mg TID; Deplin 15mg; Xanax PRN; Metoprol 100mg.

Thanks for responding.

actually, I wasn't talking about any specific drug. I was mumbling about how does a drug achieve the status of stand-alone. What doors does a drug manufacturer need to open for the FDA to approve the drug for a specific status. it was a boring question.

Not a boring question. I assume all drugs are applied for as drugs that are efficacious on their own.

I could not find a definition for "stand alone" - but I did see that term used on the Abilify site.

However, on the FDA site, I did find a definition of "Combination Products": http://www.fda.gov/CombinationProducts/ AboutCombinationProducts/ucm118332.htm

On another site, I found it interesting that it takes an average of 12 yrs for a drug to go through the FDA approval process.
